REQUEST TO FREEZE MEMBERSHIP
I am requesting to freeze my membership with Healthy Glow
- I have specified the amount of months that my account should be frozen for with a Healthy Glow consultant.
- I understand that once this freeze time frame I requested has expired, that my card will be charged the full amount of the membership price.
- I understand that freeze fees are not refundable.
- I authorize Healthy Glow and assigns to automatically charge the $5 Freeze Fee to the credit card on file.
